Five Step Overview
1. Participate in your implementation call
We can get to know one another, discuss a communication strategy, and answer your questions. Our team will be in touch to schedule this call.

Benefit consultants, HR team and Unum Account Executive.
2. Submit your plan & employee eligibility information
We need details about your ancillary benefits, your health plan documents and contribution structure, as well as a file of all employees eligible to enroll in your plans. If you have different contribution levels for different employee groups, this should be clearly identified in the file.

- Unum: Submit this form to let us know which ancillary benefits are offered.
- Benefit consultants: Typically provide plan documents. Here’s a brief list of what to send.
- HR teams: Send eligibility file using this template
3. Approve your plan-rate details
We’ll send an overview of your Plans and Rates, as they’re entered into our system. This is an important step because we cannot launch Decision Doc without this sign-off!

4. Let us do our magic 🙂
We build your plans into our system and create your custom communications strategy (you’ll use this to share Decision Doc with employees!)

This is all on us! While you’re waiting, check out some basic talking points about Decision Doc.
5. Go Live!
When it’s time, you’ll receive our “Go Live” email. This will include your final Communication Strategy and a test link to try out Decision Doc. From there, we’ll plan to send you updates every few days on how our engagements with your employees are going!
